How to Make an Appointment
To begin the appointment process, you can either call the International Relations Department directly at 813-745-3571, or you can complete one of the following forms online at the New Patient Appointment Center (NPAC). You have the option to either refer yourself or if you are a physician, you may refer a patient. Please make sure to fill out all of the required information. If any information is invalid or left blank, your registration will not be processed.
If you have international insurance payable through an office in the United States, a representative from the New Patient Appointment Center (NPAC) will verify your insurance within 1-2 business days. If you are a private pay patient, then our International Financial Counselor will contact you to discuss consultation prices, estimates and required deposits. If any additional information is required, we will contact you. An NPAC intake specialist will then call you within 3 business days and schedule your appointment.
