Patients can schedule a visit, consultation or second opinion by contacting the Blood & Marrow Transplant Program directly at 813-745-8530. A patient representative will schedule your appointment and help you gather all of the necessary information for your visit, such as medical records, X-rays, pathology slides and test results, prior to your appointment. Patients will be given specific directions on where to go in advance of their appointments.

Self-Referral
Option 1: Patients can schedule a visit, consultation or second opinion by contacting the New Patient Appointment Center at 1-888-860-2778 or 813-745-3980. A patient representative will schedule your appointment and help you gather all of the appropriate information, such as medical records, X-rays, pathology slides and test results, before you arrive.
Option 2: Complete the online New Patient Preregistration Form which will activate your new patient appointment process. Our New Patient Scheduling Specialist will call you directly to collect additional information required before confirming your appointment.
