How to Apply
We accept applications only through the Electronic Residency Application System (ERAS). Our fellowship program will also participate in the National Residence Matching Program (NRMP) Fellowship Match in 2012 for fellowships with a starting date of July 1, 2013.
Deadline for submission of applications for the 2013-2014 academic year will be October 1, 2012. Interviews will be conducted from to August - November 2012.
Thank you for your interest in the Hematology/Medical Oncology Fellowship Program at the University of South Florida. Eligibility criteria for application to the program include the following:
- MD or DO
- US Citizenship, Permanent Resident, or J1 Visa Status through ECFMG
- Completing 3 years of U.S. internal medicine residency or combined medicine/pediatric residency training that is accredited by the Accreditation Council for Graduate Medical Education (ACGME) and establishes eligibility for certification by the American Board of Internal Medicine
The application will require the following materials:
- Complete CV
- Personal Statement
- Three Letters of Recommendation (one of which should be from the residency department chairman or program director)
- Proof of Citizenship
- Medical School Transcripts
- USMLE Board Score Reports
- COMLEX Transcript
- ECFMG Certificate (if applicable)
- Recent Photograph
Additional Documents for Foreign Medical School Graduates:
- Transcript of all college education
- Dean's Letter
- Foreign Medical License (if applicable)
