Accreditation
and Designation Statements
This activity has been planned and implemented in accordance with the Essentials
and Standards of Accreditation Council for Continuing Medical Education through
the joint sponsorship of the University of South Florida College of Medicine
and H. Lee Moffitt Cancer Center & Research Institute. The University of
South Florida is accredited by the Accreditation Council for Continuing Medical
Education (ACCME) to sponsor continuing medical education for physicians.
The University of South Florida College of Medicine designates this educational
activity for 2 hours of Category 1 credit toward the AMA Physician's Recognition
Award.
The University of South Florida is ACPE approved by the American Council on
Pharmaceutical Education as a provider of continuing pharmaceutical education.
"The Geriatric Cancer Patient: Equal Benefit from Equal Treatment" has been
approved for 2.0 contact hours of continuing education credit. The Universal
Program Number is 230-000-01-013-L01.
This activity for 2.0 contact hours has been provided by the University of South
Florida College of Nursing, which is accredited as a provider of continuing
education in nursing by the American Nurses Credentialing Center's Commission
on Accreditation. Each nurse should claim only those hours of credit that he/she
actually spent in the educational activity.

the supplement and then answer the posttest questions. Fifteen or more correctly
answered questions earns 2 CE hours.
- 2. Your
answers can be returned for grading by mailing or faxing the enclosed answer
card or by answering the questions online via Moffitt’s home page at http://www.moffitt.usf.edu/ccjournal.
Upon successful completion, a certificate for the credits will be mailed to
you. Please allow 4-6 weeks to receive your certificate.

the evaluation questions on the attached CE reply card to help improve future
CE
activities.
