The completed application will contain:
- A completed application form submitted electronically in Word format to spark@moffitt.org
- At least one letter of recommendation from a person with whom you share an academic relationship
- An official transcript of grades from your institution
- 1-2 page statement of interest describing applicant's career goals and interest in health disparities research
