Pathology Department
Our mission is to provide accurate, timely and cost-effective pathology services to Moffitt patients and physicians, its referring physician and to provide consultative services. We maintain the highest levels of quality to ensure the most accurate diagnostic services for both the patient and the attending physician. The Divisions of Anatomic Pathology, Hematopathology and Clinical Laboratory Medicine are committed to assisting the Cancer Center in its mission of contributing to the prevention and cure of cancer.
The Department of Pathology is accredited by the College of American Pathologists – a mark of excellence in the practice of Pathology and Laboratory Medicine. The quality of work is continuously monitored by internal and external quality control programs, by participation in proficiency testing and by participation in hospital-wide quality improvement programs. Our goal is to offer outstanding consultation services with rapid turnaround time to our referring pathologists, physicians and patients in order to expedite the diagnosing and treatment of our patients. Through extensive investigation and research, our main focus is to contribute to the prevention and treatment of cancer in order to make a life-saving difference in our patients and their families' lives.
Pathology Records
To request patient pathology reports, please send a fax request to 813-449-8001 and include the following information:
- Patient's name
- DOB (date of birth)
- MRN (medical record number)
- DOS (date of service) - Provide estimated timeframe if possible
- Report type
- Date records are needed
