Insurance Coverage

For complex medical care such as cancer treatment, understanding your insurance coverage is important. 

With insurance plans evolving, it's important to make sure Moffitt and cancer care are part of your healthcare plan. For help navigating insurance plan selection, you can contact our Insurance Navigators at FCUinsuranceNavigator@Moffitt.org or call 813-745-7300.

List of In-Network and Out-of-Network Plans

Healthcare plans that are in-network mean that Moffitt and the plan have a formal contract in place for services provided at Moffitt facilities by Moffitt Medical Group faculty and providers. Members will have co-pays, co-insurance and an in-network, out-of-pocket maximum. 

Out of Network doesn't always mean out of reach. 

Many plans offer the ability for members to use out-of-network providers for treatment, especially for specialized hospitals like Moffitt. Check your plan's out-of-network benefits to understand the cost of going to an out-of-network provider for cancer care versus an in-network provider.  

Before contacting your insurance company, you may want to review questions to ask your insurance company. 

  • Commercial Plans (Employer Sponsored, Group Plans, or Health Exchange)

    If you get your medical insurance through your employer or from the ACA/Health Insurance Marketplace, you likely have a Commercial Insurance Plan. 

    • Aetna - Moffitt is in-network

    • Ambetter - Moffitt is in-network for Transplant Services for Ambetter Health through Celtic Transplant

    • AvMed - Moffitt is in-network for AvMed’s commercial HMO and PPO plans

    • Beech Street PPO - Moffitt is in-network for the Multiplan PPO network. If you have a different plan, please contact your insurance carrier to find out if your plan provides in-network access or out-of-network benefits

    • Blue Cross Blue Shield of Florida (Florida Blue) - Moffitt is in-network for Florida Blue’s commercial HMO, POS and PPO plans, including BlueCare, MyBlue, Traditional,  BlueSelect and Network Blue

    • Cigna - Moffitt is in-network for Cigna’s commercial HMO and PPO plans, including Cigna LifeSOURCE Transplant Network and Cigna Pathwell Specialty Network and Cigna Open Access. Moffitt it out-of-network with Cigna Disnay Orlando Health and Cigna Disney Advent Health 

    • Celtic - Moffitt is in Network with Centene's Celtic Transplant Services (Ambetter)  

    • Claritev (formerly MultiPlan) - Moffitt is an in-network provider for the PPO network. If you have a different plan, contact your insurance carrier to find out if your plan provides in-network access or out-of-network benefits

    • Curative Services, LLC - Moffitt is in-network for PPO plans

    • Emerging Therapy Solutions (ETC) Transplant Network  - Moffitt is in network

    • Employers Healthcare Network - Moffitt is in-network for PPO plans

    • Evolutions Healthcare System - Moffitt is in-network for Prime and Select PPO networks. If you have a different plan, contact your carrier to find out if your plan provides in-network access or out-of-network benefits

    • First Health Network (owned by Aetna Healthcare) - Moffitt is in-network for the First Health PPO network. If you have a different plan, contact your insurance carrier to find out if your plan provides in-network access.

    • Florida Blue - Moffitt is in-network for Florida Blue’s commercial HMO, POS, and PPO plans, including BlueCare and MyBlue, Traditional, and BlueSelect

    • Gulf Coast Provider Network - Moffitt is in-network for the Gulf Coast Provider Network PPO. If you have a different plan, contact your insurance carrier to find out if your plan provides in-network access or out-of-network benefits

    • Health First Health Plan - Moffitt Cancer Center is in-network for all Health First Plans commercial and Affordable Care Act plans.

    • Interlink - Moffitt is an in-network provider for Interlink’s National Transplant and Cancer Care Networks

    • LifeTrac Transplant Network - Moffitt is in-network for LifeTrac’s National Transplant Network

    • Neighborhood Health Partnership - Moffitt is in-network for HMO

    • Optum - Moffitt is in-network for Optum’s Transplant and Cancer Resource Services (CRS) Networks

    • Private Healthcare Systems (PHCS) - Moffitt is in-network for PPO

    • Quality Health Management - Contact your insurance carrier to find out if your plan provides in-network access

    • South Florida Community Care Network d/b/a Community Care Plan, a Medicaid Managed Care

    • United Healthcare   - Moffitt is in-network for United Healthcare’s commercial HMO, POS, and PPO plans, including Neighborhood Health Partnership HMP/POS plans. 

    • UMR - Moffitt is in-network for UMR’s commercial HMO, POS, and PPO plans

    • Unity Preferred - Moffitt is in network
    • Velocity National Provider Network - Moffitt is in-network 

    • Volusia Health Network - Moffitt is in-network for Volusia Health Network

  • Government Payers (Medicare, Medicare Supplement, Medicaid, VA Benefits, Government Employees)

    Moffitt accepts Traditional Medicare and Medicaid. In addition, Moffitt is in-network with all of Florida's Managed Medicaid plans. Moffitt is in-network with TRICARE and VA Community Care.  Moffitt accepts ChampVA and Hillsborough County Health Plan.

    • Blue Cross Blue Shield Supplements -  Moffitt is in network with most BCBS Medicare Supplements, except for the BCBS Advantage65 Select Medicare Supplement.   

    • ChampVA - Moffitt is in-network

    • Community Care Plan Medicaid - Moffitt is in-network with Community Care Plan Medicaid

    • Department of Labor Office of Workers Compensation Programs (OWCP) - Moffitt is in-network

    • Florida Community Care -  Medicaid HMO is in-network 

    • Hillsborough County Health Care Plan - Moffitt is in-network for Neuroendocrine, Sarcoma, Endocrine, Acute Leukemia, Triple Negative Breast Cancer, and Mesothelioma cases only. Contact your insurance carrier to learn if your plan provides in-network access.

    • Health First Health Plan - Moffitt Cancer Center is in-network for Medicare plans

    • Hillsborough County Health Care Plan - (for Neuroendocrine, Sarcoma, Endocrine, Acute Leukemia, Triple Negative Breast Cancer, and Mesothelioma only). Contact your insurance carrier to learn if your plan provides in-network access or out-of-network benefits

    • Humana - Moffitt is in network with Humana's Medicaid HMO

    • Humana Transplant - Moffitt is in network with Humana's Transplant network for Medicare and Medicaid  
       
    • Interlink Medicaid - Moffitt is in network
    • Medicaid - Moffitt participates in Medicaid. Moffitt is in-network with all Florida Medicaid plans.

    • Medicare - Moffitt participates in Original Medicare.

    • Medicare Supplement/ Medigap - Moffitt accepts hundreds of Medicare Supplement plans nationwide. Please provide your secondary insurance information to our team when you register as a new patient. 

    • Molina Healthcare - Moffitt is an in-network provider for Molina’s Medicaid HMO and Serious Mental Illness (SMI). 

    • Optum VA Community Care Network (CCN) - Moffitt is in-network with the VA CCN

    • Sunshine Health -  Moffitt is in-network with Sunshine Health Medicaid  

    • Simply Healthcare - Moffitt is an in-network provider for Simply Healthcare’s Medicaid HMO and Simply Clear Health Alliance. Moffitt is out-of-network for Simply Healthcare’s Medicare HMO.

    • TRICARE - Moffitt is in-network for TRICARE Prime, TRICARE Select, and TRICARE for Life. 

    • United Healthcare Medicaid -  Moffitt is in-network with UnitedHealthCare Medicaid  

    • World Trade Center - Moffitt is in network for programs administered by Sedgewick and NYFD

  • Medicare Advantage Plans

    Out of network doesn't always mean out of reach.  

    As of June 30, 2026, Moffitt is out of network with many of the largest Medicare Advantage plans. However, more than 13,000 patients came to Moffitt to receive cancer care using their out-of-network benefits with a Medicare Advantage Plan in the last 12 months.

    What should I do if my insurance plan is not in network with Moffitt?

    Ask your insurance plan about your out-of-network benefits. For Medicare Advantage PPO plans, you can access Moffitt via those benefits. For some plans, the total out of pocket cost is the same whether you get cancer care in network or out of network. 

    Ask your Doctor for a Referral to Moffitt. For cases needing specialized care, Moffitt will attempt to secure an authorization with your insurance plan. We need a referral from your physician (primary care, oncologist or specialist) to start that process.

    • Elevance -  Freedom, Simply and Optimum Plans:  Moffitt is out of network with Elevance Medicare Advantage Plans  

    • Florida Complete Care - Medicare Advantage HMO is in-network.

    • Gold Kidney of Florida -  Moffitt is in-network

    • Health Care Service Corporation - Moffitt is out-of-network for Medicare Advantage HMO and PPO plans. Please contact us or your plan for information on options for using out-of-network benefits at Moffitt.

    • Humana -  Moffitt is in-network with Humana's Transplant Services. Moffitt is out-of-network for Humana's Medicare Advantage HMO and PPO plans. Please contact us or your plan for information on your out-of-network benefits.

    • United Healthcare - Moffitt is in-network with Optum Transplant Services. Moffitt is out-of-network for United Healthcare’s Medicare Advantage HMO and PPO plans. Please contact us or your plan for information on your out-of-network benefits. 

    • WellCare - Moffitt is out-of-network with WellCare. Moffitt is in network for transplant services under Centene's Celtic Transplant.

Moffitt Cancer Center plays a proactive role in helping patients receive quality cancer care by working with an extensive national network of insurance and managed care organizations. Moffitt will work with patients to obtain the necessary insurance authorization to receive treatment, although patients are ultimately responsible for adhering to their healthcare insurance plan.

No billing surprises. All insurance is billed by Moffitt Cancer Center and by Moffitt Medical Group. All providers that you see at a Moffitt facility are part of the Moffitt Medical Group; you will never receive a bill from an outside provider.  

Many managed care plans must meet specific requirements before consultation and treatment at Moffitt Cancer Center. Please check with your insurance company for any special instructions.
