Applications for the Fall 2025 VolunTeen program will be open from August 4, 2025 to 4:30 p.m. August 21, 2025.

Due to the nature of Moffitt’s environment and the requirements for volunteering, teens will be required to commit to a minimum of two months of volunteer service, serving one three-hour shift per week. 

The Fall 2025 VolunTeen program will run from September 14, 2025 to November 8, 2025. Assignments will be offered in the evenings and weekends to accommodate teen schedules. Shifts will be from 5 p.m. to 8 p.m. on Monday through Friday and from 8 a.m. to 11 a.m. on Saturdays and Sundays.

To be considered for the program, teens must:

  • Be between the ages of 15 and 17 years old.
  • Commit to volunteering for the full session.
  • Submit an electronic application and all required documents (see below) before 4:30 p.m. on August 21, 2025.
  • Attend a mandatory, in-person orientation on Saturday, September 6, from 7 a.m. to 2 p.m. (location details to follow).

The following Occupational Health Documents are required to be submitted to vsdocuments@moffitt.org no later than August 21, 2025.

1. Submit a Copy of Your Immunization Record(s)

A copy of your immunization record can be obtained from places such as a school, previous employer, your personal physician (pediatrician), or your state’s public health department. All vaccinations administered in the state of Florida are recorded in Florida Shots, and you may request a copy of your vaccine records on their website. Please be aware that it can take up to a week to obtain the requested record.

What we will be looking for from the immunization record:

  • Proof of two (2) MMR vaccines (Measles, Mumps & Rubella) OR bloodwork documentation showing immunity.
  • Proof of two (2) Varicella (Chicken Pox) vaccines OR bloodwork documentation showing immunity.
  • Proof of a completed Hepatitis B vaccine series OR bloodwork documentation showing immunity, AND submit signed Hepatitis B Declination Form (Please refer to Section D below).

2. Submit Proof of a Negative QuantiFERON (QFT) Blood Test Results Within One Year.

    • TB Skin test will NOT be accepted.

3. Sign and Submit- Hepatitis B Declination Form

  • The Hepatitis B Declination form is required by OSHA and must be recorded in your Employee/Volunteer Health Record.
  • The declination form must be signed by a legal parent or guardian if the participant is under the age of 18.

Final acceptance into the program depends on submission and clearance of all required documentation and attending the mandatory in-person orientation.

Admission into the program is first-come, first-served and based upon completing all the mandatory onboarding requirements. Once capacity has been reached, we will notify the remaining applicants to apply for the Spring 2026 program.
