Medical Records

Patients can access their medical records at any time through the Patient Portal. Most medical records are available immediately as soon as they are finalized. This aligns with changes mandated under the 21st Century Cures Act.

Contact Your Care Team: If you have any questions or concerns after reviewing your records that require immediate attention before your next clinical or virtual visit, please reach out to your clinical care team. They can help you understand the findings in your records.
Exceptions: Florida law provides extra protections for certain sensitive records, including
psychiatric and genetic test results. If you need access to these specific records, please request
them directly from the Medical Records team.

What do I tell my physician, who needs to send my medical records to Moffitt?
Your referring physician should also contact the specific clinic and find out exactly what documentation they need. If a patient or their referring physician needs their Moffitt medical records to be sent out, they would contact ROI (Release of Information) at 813-745-3991.

What is the Tampa Bay Health Information Exchange (HIE)?
The Tampa Bay Health Information Exchange (HIE) was developed to assist in the sharing of protected health information among healthcare facilities in the Tampa Bay area. The HIE is a secure, electronic service that allows the sharing of health information between doctors’ offices, hospitals, pharmacies, labs, radiology centers, and other healthcare organizations. The HIE is a member of the Florida Health Information Exchange. It is also called Sunshine Health Connect.
