Sending Medical Records to Moffitt

Your Moffitt care team will review any outside records prior to your first appointment. Your assistance in obtaining the records to help our providers prepare is greatly appreciated. 

How to obtain your medical records from other facilities

There are a few ways to obtain your medical records from a hospital or a doctor’s office.

1. If your doctor’s office has a patient portal, you may be able to download your records to your computer from there.
   
   
2. Visit your doctor's website and search for a medical record release form (may also be known as a request for access).
   
   
3. Contact your doctor’s office via phone to request the records directly.

The records can be sent directly to Moffitt. Find your clinic or cancer type below and send your records to the corresponding email or fax number. 

What medical records should you send?

Please submit all records listed below, as well as any other records specific to your visit.

• Pathology report pertaining to your diagnosis, if a biopsy has been performed
  
  
• Radiology reports and CDs within the last 5 years for any imaging performed (Mammogram, Ultrasound, MRI, Staging Images, CT, PET, Bone Scan etc). 
  
  
• Most recent office visit notes from your referring doctor
  
  
• Previous medical history such as prior operative procedures, cardiology tests, lab work (CBC, CMP, BMP), genetic testing or tumor markers, and previous chemotherapy and/or radiation treatment notes
