What happens next?
Our team will verify the information you provided on the request form.
- Based on the insurance plan, most patients will be cleared for scheduling within 1 business day. Some insurance plans require additional steps. If this is the case, we will be in contact to walk you through the process.
- Once your insurance is verified, a patient access scheduler will reach out to schedule your appointment. Most patients will hear from their scheduler within 1-2 business days.
Throughout the process, if our team has any questions, they will contact you via phone call, text or email.

How to prepare for your scheduling call
During your scheduling call, you'll be asked a few questions about your medical history so we can ensure you're matched with the best care team for you. To make the process as smooth as possible, please gather any available information about your care history, including:
- The names of facilities where you have received care
- Referring and treating physician names
- Facility addresses and phone numbers (if available)
- Dates or approximate timeframes for any:
- Imaging
- Laboratory testing
- Pathology
- Surgeries
- Cancer treatments
