On MyMoffitt patient portal, you can:
- Complete your first appointment questionnaire
- Find all your appointment times and appointment instructions
- Review and request updates to your health information
- Request prescription renewals
- Ask non-urgent health-related questions
- View most medical records and lab results as soon as they are finalized. This aligns with changes mandated under the 21st Century Cures Act
- View pathology results four days after they are completed by the pathology team
- View and pay your hospital bill and schedule payments
- Access helpful educational materials
- Upload your outside medical images (radiology, cardiology, endoscopy, etc.)

What do I need before I enroll in the patient portal?
You must be a registered patient with Moffitt Cancer Center and have your Moffitt Medical Record Number. Access to an email account is required. You can sign up for a free email account at a number of websites including www.gmail.com, www.outlook.com, or www.ymail.com.

How do I request copies of my medical records?
Patients can request copies of their medical records by completing an Authorization for Use or Disclosure of Protected Health Information form electronically through the MyMoffitt Patient Portal, by downloading and completing the form from Moffitt.org, or making a request in person.
- MyMoffitt Patient Portal: log in with your account and select Records Request located under Additional Links.
- Visit Moffitt.org and click on the link to the Authorization for Use or Disclosure of Protected Health Information.
