To help us prepare, please request accommodations 5-7 business days before your appointment.
How to Request an Accommodation/Modification
All accommodations can be requested by using our Accommodation Request Form or by emailing AccessCoordination@Moffitt.org. There are several categories to choose from:
- Routine: 6 or more business days
- Time-Sensitive: 3-5 business days
- Urgent: 2 or fewer business days

- Complaints and grievances are handled through our Patient Relations Office.
