cancer survivor speaks with healthcare professional

The Blood and Marrow Transplant and Cellular Immunotherapy Survivorship Clinic is part of the BMT-CI program at Moffitt Cancer Center and focuses on the part of your journey after transplant or CAR-T therapy. The goal is to help patients navigate ongoing medical monitoring, to address potential complications, late effects and other health concerns that may arise years after treatment.

In the survivorship clinic, we evaluate your individual medical and treatment history to develop a personalized care plan for the future. The survivorship clinic visit is not a group visit and is also not a support group.

What is a care plan?

Your care plan will include a summary of all of the prior treatments you have received and will provide individualized recommendations for check-ups and screenings that can help in the early detection of complications following these treatments.

How often will I be seen?

You will be seen for an initial visit, a six-month follow-up, and then yearly thereafter. Some patients may be seen more or less frequently depending on individual needs.

How long will I be seen for?

Most patients are seen for a yearly check-up.

How do I get a survivorship visit?

Your primary BMT-CI physician at Moffitt can refer you for your initial visit starting as early as 100 days after transplant or 12 months after some cellular immunotherapies, including CAR-T therapy.

Who will be my care team during the visit?

Your care team includes advanced practice providers and transplant/CAR-T physicians who have experience and specialized focus in survivorship care. You may also be seen by other medical specialists as needed.

Do I need labs or diagnostic studies prior to survivorship visits?

Generally, there are no labs or diagnostic studies ordered with your initial survivorship clinic visit. However, studies may be ordered after the visit depending on your history.

Can a survivorship visit be held virtually?

All visits are available virtually.

Do I have to do virtual care?

In-person visits are also available.
